Preparing your healthcare facility for accreditation requires a clear understanding of the official NABH accreditation handbook guidelines. Established under the Quality Council of India (QCI), the National Accreditation Board for Hospitals & Healthcare Providers (NABH) is the apex body driving patient safety and healthcare quality standards across Indian hospitals.

Effective January 1, 2025, the newly notified 6th Edition of the NABH Hospital Standards replaces the legacy 5th Edition. In this 6th Edition handbook, the total number of objective elements has been pruned down to 639 objective elements. These elements are structured strategically into ten chapters and categorized into four performance tiers: Core, Commitment, Achievement, and Excellence.

To achieve compliance, hospitals must implement standard operating procedures (SOPs) matching these elements. Specifically, the 105 Core Category elements are mandatory and will be assessed at every audit cycle. These include critical workflows like patient risk registers, triage protocols, LASA (Look-Alike Sound-Alike) drug verification, fire safety drills, and structured Information Management Systems (IMS).


Core Objective Elements vs. Levels

The 6th Edition outlines a quality journey. Beyond the mandatory 105 Core elements, hospitals are evaluated against 457 Commitment elements during the final assessment, 60 Achievement elements during surveillance (21-24 months), and 17 Excellence elements during re-accreditation (valid for 4 years).

Transitioning to Digital EMR Audits
